Patient Referral
For our practice to arrange the next available appointment for you or your patient please provide the referral by completing the following form below.
Patients referrals may also be delivered to the Sleep and Lung Centre by medical objects, fax or email.
What to bring to your appointment
- Your GP or specialist referral
- Any chest, CT/imaging scans that you have had taken within the last 6 months
- Copies of your recent sleep study or CPAP download if applicable
- A current medication list and any allergies that you may have
- Your Medicare Card
- Your Private Health Care Card or Veterans Affairs Card if you have one
Inpatient referrals
Our Physicians have admitting rights at Greenslopes Private Hospital. For patients needing inpatient care, the referring doctor can contact our staff on 3324 1699 to arrange inpatient reviews with our Physicians.
Urgent referral
Urgent referrals may be arranged by contacting our friendly reception staff on (07) 3324 1699 (9am to 5pm, Monday to Friday).
Urgent consultations are usually seen within the next business day.
